Toolkit for SPARC Partners
SPARC (Sickness Prevention Achieved through Regional Collaboration) brings together community agencies serving older adults to provide health screenings, immunizations, and other preventive services in convenient locations. ARC is pleased to provide the following materials for partner agencies to use in organizing, promoting, and reporting results of SPARC events.
SPARC Press Release Template (DOC) - Customize template and distribute to local news media to promote your SPARC event.
SPARC Poster (DOC) - Drop in dates, times, and locations of your SPARC events.
SPARC Evaluation and Survey Forms (below) - Evaluation of your SPARC event is mandatory. Please make certain that someone is gathering this information during the event and then send a copy of the completed forms to Jessica Gill at ARC at 40 Courtland St., NE, Atlanta, GA 30303.

SPARC Primary Care Referral Resources (below) - Follow-up with a medical provider is very important should someone participating in the SPARC event receive abnormal results. Attached are the referral lists of health centers that provide primary care services and hospital based physician referral programs.

Personal Prevention Record (PDF) - Print and give to SPARC participants to help them keep a record of important health information.
Educational Presentation for SPARC Participants (PPT) - If desired, download the PowerPoint presentation and run on a continuous loop at SPARC events.
